REQUEST FOR QUALIFICATIONS
Owner Representative/Project Manager
The Dallas County R-I School District, hereafter referred to as the District, is soliciting written proposals from qualified firms and individuals to provide professional owner representative/project manager services for the following project:
Dallas County Technical Center construction on the West side of Buffalo High School, totaling approximately $12.0 million in construction costs.

Respondents shall submit their proposal via mail or delivery to Dr. Tim Ryan, Superintendent of Schools, 1323 S. Ash, Buffalo, MO no later than 12:00pm on Tuesday, March 17, 2020. Proposals received after the aforementioned date and time may not be considered.

Proposals shall include at minimum:
1. Company profile and history including:
a. Number of years in business
b. Number of years under present business name
c. If incorporated, date of incorporation, State of incorporation, and names of officers
d. If partnership, date of organization, type of partnership, name(s) of general partner(s)
e. If individually owned, date of organization, name of owner
2. Qualifications, education, and experience of the proposed project management team, as well as other supporting personnel
3. Qualifications and approach to construction-phase services, including:
a. Schedule adherence
b. Cost control
c. Job-site coordination
d. Change order management
e. Site control and safety
4. Experience in the construction of projects of similar scope and size a. List all that apply
b. Give explanation of projects completed within past 5 years, including owner, architect, contract amount and date of completion
5. Experience in educational facility projects.

6. Proof of insurance and State of Missouri contractor’s license
7. Claims and Suits (If “yes” to any of the following questions, please explain)
a. Has your organization ever failed to complete any work awarded to it?
b. Are there any judgments, claims, arbitration proceedings or suits pending or outstanding against your organization or its officers?
c. Has your organization filed any lawsuits or requested arbitration with regard to construction contracts within the last five years?
8. References, including
a. Name, email and phone number of current and past clients
b. Name and phone number of bank references
c. Name and phone number of bonding company and agent
